Appointments will operate on a first-come, first-served basis, so you may wish to arrive promptly on the day to avoid a wait to see Nus.
In order for Nus to be able to help you as efficiently as possible with your queries and request for surgery appointments, please make sure you have included the following information in the attached form:
- Your name
- Full residential address including postcode
- What steps you have taken to try to resolve the problem yourself
- What you believe I can do to help you
- Any relevant paperwork
- For Home Office queries, please forward your Home Office correspondence ID number
- Date of Birth
